                   Behavioral Health Program for Outpatients
                   Program Procedure: Document Services
 

Objective 6: Document services provided and submit to funding source for reimbursement.

 Each client contact session will be listed by client identifier number, date of session, time in session, and the acknowledgment that progress notes were completed. The length of time in session must match the time in session indicated in the SOAP notes. Each SOAP note must indicate whether the session was an individual session, individual evaluation session or a group session for billing purposes. Services provided for youth (under 19 years of age) must be documented on  a separate session form and billed on the BH3 form.

 Activity 1: Submit a monthly report of services provided to the Clinic Manager.
