Business Associate a
person or organization that, on behalf of NUIMC, performs or assists in the
performance of an activity which involves the use and/or disclosure of
protected health information. This use/disclosure may include but is not
limited to: billing, quality assurance, data analysis, and/or providing a
medical service
Correctional Facility
any penal or correctional
facility, jail, reformatory, detention center, work farm, halfway house, or
residential community program center operated by, or under contract to, the
United States, a State, a territory, a political subdivision of a State or
territory, or an Indian tribe, for the confinement or rehabilitation of
persons charged with or convicted of a criminal offense or other persons
held in lawful custody
Covered Entity a health plan, health care clearing house, or any health
care provider who transmits health information in electronic form
Covered Function functions of a covered entity the
performance of which makes the entity a health care provider
Data Aggregation with respect to protected health information created or
received by a business associate in its capacity as the business associate
of a covered entity, the combining of such protected health information by
the business associate with the protected health information received by the
business associate in its capacity as a business associate of another
covered entity, to permit data analyses that relate to the health care
operations of the respective covered entities
Disclosure release, transfer, provision of access to, or divulging in any
other manner of information outside the entity holding the information
Health Care Operations broad range of business and administrative
activities of a covered entity, including, but not limited to: quality
assessment/improvement activities, education and training of
students/volunteers, reviewing the competence or qualifications of health
care professionals, contracting for health care services, and business
planning/management

Individual the person who is the subject of protected health information
Individually Identifiable Health Information information that is a subset
of health information, which may include demographic information, which
identifies the individual
Legal Health Record the portion of the chart we may release; includes, but
is not limited to: patient demographic information (date of birth, address,
phone number), history and physical data, nurses notes, progress notes, lab
notes/results, and billing/insurance information.
Payment activities undertaken to obtain or provide compensation or
reimbursement for the provision of health care services
Personal Representative any person authorized under applicable law to act
on behalf of the individual patient with respect to the individuals health
care
Protected Health Information any individually identifiable health
information used by or disclosed to a covered entity that is oral, recorded
on paper, or sent electronically
Research systematic investigation, including research development,
testing, and evaluation, designed to develop or contribute to generalizable
knowledge
Required by Law a legal mandate that compels a covered entity to make a
use or disclosure of protected health information and that is enforceable in
a court of law
Treatment - provision, coordination, or management of health care and
related services by one or more health care providers, including the
coordination or management of health care by a health care provider with a
third party; consultation between health care providers relating to a
patient; or the referral of a patient for health care from one health care
provider to another
Use with respect to individually identifiable health information, the
sharing, employment, application, utilization, examination, or analysis or
such information within an entity that maintains such information
