Policy
It is the policy of Nebraska Urban Indian Medical Center to
transport persons to NUIMC appointments
as well as to transport persons to
other specific appointments as approved by the Transportation Supervisor.
The Nebraska Urban Indian Medical Center reserves the right to bill patients
for trips to/from outside
the Lincoln area. NUIMC will not discriminate
against persons requesting transportation; however, Indian
preference will
be given.
Procedure
-
A patient requesting
transportation must do so 24 hours prior to the time/date of appointment.

A patient requesting
transportation may do so by calling the Transportation Supervisor or
Transportation Specialists. The transportation supervisor and specialists
will be responsible for
maintaining their own transportation schedules.
- Any
patient requesting transportation will be accepted under the following
conditions:

Must be a patient of
NUIMC
-
Must be transported to a
medical appointment, WIC, or Medicaid in Lincoln or Omaha
-
Must live within a
45-mile radius of NUIMC
- The
Transportation Supervisor and Transportation Specialists reserve the right
to deny a patient
transportation based on the following:

Lack of availability
-
History of
transportation no-shows
-
Indian preference
-
The Transportation
Supervisor and Transportation Specialists will verify the name, address,
home phone
number, as well as date, time, and location of appointment with
the patient when they call to
arrange transportation.

The Transportation
Supervisor and Transportation Specialists will pick the patient up at
their residence
15 to 60 minutes prior to their appointment time and take
them to their appointment. After the appointment,
they will pick up the
patient at the appointment location and return them to their residence.

There is a No Smoking
policy in all GSA automobiles, and this policy is posted in a visible
place in the vehicle.

The Transportation
Supervisor and Transportation Specialists will ensure that Nebraska state
laws
regarding seat belts and child restraints are strictly adhered to.

The Transportation
Supervisor and Transportation Specialists will keep mileage and activity
logs on each
patient transported. These mileage logs include: patient’s
age, ethnicity, and where they were transported
to. These mileage logs
will be kept daily and a monthly report will be submitted to the Executive
Director
by the Transportation Supervisor.

The Transportation
Supervisor will be responsible for driving to Winnebago, Nebraska monthly
to
pick up prescription medication for Native American patients. The
Transportation Supervisor will
ensure that Winnebago pharmacy has the
prescriptions at least 48 hours prior to pick-up.

The Transportation
Supervisor and Transportation Specialists will carry valid automobile
insurance and
their driver’s license with them at all times.

The Transportation
Supervisor and Transportation Specialists reserve the right to charge a
$10.00 fee for
all trips outside the Lincoln-Lancaster County area.

The Transportation
Supervisor will be responsible for projecting monthly numbers of how many
patients
the program will serve. These projected numbers will be compared
with actual numbers and submitted
with the monthly report. Depending on
these numbers, the Transportation Supervisor will determine
workload
schedules of the Transportation Specialists.

Accidents will be reported
to the proper authorities (GSA, Police Department, NUIMC Executive
Director, etc.)
by the Transportation Supervisor in a timely manner.

The Transportation
Supervisor will be responsible for administering semi-annual patient
satisfaction surveys
to monitor the patient’s satisfaction with the
program. The Transportation Supervisor will report the findings
to the
Executive Director and will be responsible for making the appropriate
changes in the program to
maximize patient satisfaction.

The automobiles used for
transportation services are taken in for routine checks dependant on the
warranty
of the vehicle.

Liability insurance will
be maintained on all transportation vehicles at all times.
Transportation Program Plan
The Nebraska Urban Indian Medical Center’s Transportation
Program serves to provide the following
services to NUIMC patients in the
Lincoln-Lancaster County area:
1.
Provide transportation for NUIMC patients
to medical appointments at NUIMC
2.
Provide transportation for NUIMC patients
to other appointments as approved by the
Transportation Supervisor, such as
WIC and Medicaid appointments
3.
Provide transportation for NUIMC patients
to medical appointments at other medical facilities
in the Lincoln-Lancaster
County area if the patient is referred by a NUIMC provider
4.
Provide transportation for NUIMC patients
to medical appointments in Omaha if the patient is referred
by a NUIMC
provider
