      Epidemiological Services Policy and Procedure Manual

 

Policy

It will be the responsibility of the Clinic Manager to make reports of diseases to the appropriate health authorities.

 Procedure

  1. The following is a list of diseases which will be reported according to the “Rules and Regulations Concerning the Reporting and Control of Communicable Diseases”:

     

    1. Immediately by phone to the Lincoln – Lancaster County Health Department:

            Plague

            Cholera

            Typhus Fever (Louse-borne)

            Yellow Fever

            Relapsing Fever (Louse-borne)

            Smallpox

 

    1. Report within 24 hours to the Lincoln – Lancaster County Health Department:

            Acquired Immune Deficiency Syndrome (AIDS)

            Amebiasis (Amebic Dysentery)

            Anthrax

            Aseptic Meningitis

            Atypical Myobacterial Infection

            Botulism

            Brucellosis (Undulant Fever)

            Campybacter

            Chancroid

            Chlamydia (Genital Infection)

            Diptheria

 

  1. All records related to reports will be kept confidential and will not be open to public inspection.

 

  1. The Clinic Manager will compile a record of the reports made and submit the report to the Executive Director on a monthly basis.
